The cost of subsequent visits will vary according to the nature of the consultation and the time involved. Medical insurance plans usually do not cover infertility and its causes, therefore the office cannot bill any medical insurance plan. For your convenience, payments may be made in cash, by check, money order, ATM or charge card (Visa, MasterCard and bank debit cards accepted). It is the patients responsibility to submit receipts to her medical insurance carrier requesting reimbursement. For each visit our office will issue a receipt stating the diagnosis, and will assist in filling out the pertinent reimbursement forms.
